Eligibility

1) Is your business located in one of the 22 North Texas counties? (Check list below)

2) Does your small business have 2-50 employees?

3) Is your business a member of one of the affiliated participating chambers in the co-op? (Check list below)

If you can answer yes to ALL 3 qualifications, then you are eligible.

Eligible agents must both be located within the participating counties and be members of an affiliate chamber. Plans provided through United Healthcare.
